これはリングフォームの改造例です。
このほかにも自分のＨＰに合うように改造してくれて結構です。
ただし、ジャンプ先を変えるのは絶対にしてはいけません。
ここにあるものは少し変更が必要です。
「id=」の=の後にメールに書いてあった自分のＩＤを半角で付け加えてください。
（たとえば、ＩＤが１の場合「id=1」とします。）
